About Us

ActiMed provides Outpatient, Home Health, Hospice, or contract Physical & Occupational Therapy in areas surrounding our OP clinics in Puckett & Seminary. Our Puckett clinic is to the Left of Dollar General.

ActiMed was formed in 2014 as a contract therapy Company. Since then, we have provided contract services to the Acute Hospital, Swingbed, Inpatient Rehab, Skilled Nursing (SNF), Long Term Care (LTC), Outpatient, School, Assisted Living Facility, and Home Health Settings. We opened our first Outpatient Clinic - ActiMed in Seminary during June 2018. ActiMed is focused on outcomes and seeks to provide the best Occupational, Speech, & Physical Therapy in Seminary, Puckett, and surrounding areas. Our commitment is to you, your family, and your facility. We will get results!

Through Physical Therapy, Occupational Therapy, and/or Speech Therapy, ActiMed can treat nearly any condition such as: Fall Risk, General Weakness, Balance, Gait, Stair Training, Pain, Joint Replacement, Joint Stability, Edema, Tendonitis, Sports Injury, Massage, Cognitive/Behavioral Deficits, Cardio/Pulmonary Conditioning, Handwriting, Vision Coordination, Vestibular Impairment, Difficulty Dressing/Feeding/Toileting, Swallowing, Speech, Language, Cognitive-Communication, Voice, Fluency, and other related disorders.
